Pour milk and whipped heavy cream in a sauce pan and heat over medium-high heat for some time. Be careful to avoid boiling

In a large bowl, whisk sugar, cocoa and grated milk chocolate. Slowly add this dry mixture into cream mixture by stirring it continuously. When mixed well, reduce the flame to medium

Mix in vanilla and sweetened condensed milk and stir until combined completely. Simmer it over low heat for 5 minutes
